CPR CLASS PAYMENT & TERMS

ATTENTION:

These Terms Only Apply to Employees of Maxim Healthcare Services Albany

You were directed to this page because you had attended a CPR class at Maxim in Albany.

The cost of the class was $55 on site.

Payment of $55 was due at the end of class.

Maxim Healthcare Services of Albany employees are granted a discount off the regular price of $75.

  • Please note that in cases of non-payment:
  • After 30 days, you will be assessed a fee of $10 for administrative cost.
  • After 60 days, you will need to pay balance ($65) and attend a new test at $25.
  • Important:
  • After 90 days, your account will be flagged for the balance & your card/certificate will no longer be eligible to be issued. You will need to find another provider at your own cost.

BY MAKING PAYMENT YOU AGREE TO THE TERMS AND CONDITIONS.

Terms & conditions:

  • Mitchell & Lexi Donovan, along with KC2MAT.COM are independent contractors & sole proprietors.
  • Services provided to employees & prospective employees of Maxim Healthcare Services of Albany are part of an existing agreement with Maxim Healthcare Services of Albany & Mitchell & Lexi Donovan.
  • Mitchell & Lexi Donovan are not employees of Maxim Healthcare Services & does not represent Maxim Healthcare Services in any way.
  • Payments accepted by Mitchell & Lexi Donovan are for services performed based on each person in attendance of class as an individual.
  • All Discounts, Fees, & Assessments are Included in the Original Price & Sale when paid upon the class date.
  • An administrative fee of $10 will be added after 30 days time of non-payment .
  • Certification Cards are issued by a third party provider on Mitchell or Lexi Donovan's authorization once a trainee has completed the cognitive exam & psycho-motor evaluation as prescribed by that provider.
  • Certification Cards are 'released' to be issued by the third party provider upon payment to Mitchell Donovan.
  • Certification Cards are issued by the class date.
  • Certification Cards are withheld by Mitchell Donovan until payment is made.
  • NO CARDS WILL BE ISSUED AFTER 60 DAYS WITHOUT A RE-TEST!
  • Re-tests after 60 days are only accepted if the original balance is paid in full.
  • Client/Trainees will be assessed a re-testing fee of $25 in addition to the original balance.
  • WITHHELD CERTIFICATION CARDS WILL NOT BE ISSUED AFTER 90 DAYS.
  • Client/Trainees will be 'dropped' from the roster & will need to make arrangements with another provider if payment is not made within 90 days.
